Substandard medicine or Doctor's Negligence...Who to Blame???

These are few pages from PIC patient register.....which clearly shows what medicine each patient was given....it proves whose negligence caused the deaths of over 100 people...If medicines were deadly, why no deaths were reported from other places in the country.....
